Appointment Information

First Visit
Although a physician referral is not required, some insurance companies require a referral from your primary care physician. Our office highly recommends that you are first seen by your primary care physician and referred to our office if needed. New appointments are typically scheduled through the primary care physician's office.

Prior to your first appointment, each new patient is asked to complete a series of forms. This will help our medical staff make a prompt and accurate assessment during the first visit. Please bring your current insurance cards, including Medicare cards, and any current prescription bottles. Please arrive 20 minutes before your scheduled appointment to complete registration and other paperwork, or print and complete the online forms before your arrival to save time.

Follow-Up or Routine Visit
For a follow-up, post-surgery, or routine visit, please arrive 15 minutes before your scheduled appointment. Always bring your insurance cards, including Medicare cards and all prescription medications. Notify the office staff of any changes to these items.

If surgery is recommended, our staff will be happy to handle all scheduling aspects of the patient's surgery, including insurance authorization.

Cancellation Policy
Please call to cancel any appointments you will be unable to keep at least 24 hours in advance so we may offer these appointments to other patients in need of treatment.
