A. For new patients, please print and complete the new patient forms from our website. For ALL patient visits, please bring your current insurance or Medicaid cards, plus any prescription medication bottles.

Q. Can I pay my past due balance at my appointment?
A. Yes, we accept personal checks, Visa, MasterCard and Discover cards.

Q. How early do I need to arrive for my appointment?
A. Please arrive approximately twenty (20) minutes early for a new apopintment and fifteen (15) minutes early for a follow-up appointment so we may verify and update any changes to your personal or insurance information.
